由申请人亲自签署的个人陈述，说明目前资产的积累过程，包括工作收入、企业收入、
投资收入、房产增值收入以及继承、赠予及非银行借款。
 如为个人借款，必须出具证明信，说明贷款人给予借款的经济能力，以及贷款人提
供借款的原因；
 如为公司借款，必须提供公司的经济实力证明其提供借款的能力，包括营业执照、
宣传册、以及在适当的情况下提供审计报告，并陈述提供借款的原因；
 对于上述借款，尽可能提供关于借还款的银行纪录。
随行家庭成员身份证明
7） 家庭状态证明：包括结婚证、配偶死亡证、离婚证、收养证、子女合法的监护或出
生证明、签署的Appendix – Declaration by spouse or de facto spouse、户口本等
8） 主申请人及随行家庭成员的护照（有效期至少12 个月）首页及签字页复印件，主申
请人及配偶的身份证复印件
教育经历
9） 主申请人的最高学历学位证明，如为本科以上学历，提供本科及以上所有学历学位
证书。
2． 随行子女
1） 年龄超过22 岁的子女必须提供全日制在学证明
2） 如因离异等原因，子女的亲生父母一方不包含在移民申请范围内，必须签署对子女
移民授权或无异议声明的公证
3． 魁北克居住或亲属
1） 在魁北克经商（限主申请人）、学习或就业证明
2） 家庭成员在魁北克的居住证明
4． 短期净资产
1） 银行存款：截止申请前至少有6 个月记录
2） 活期账户：目前移民官比较关注资产是否完全申报，因为关系到维持家庭日常消费
的银行账户问题
3） 房产：房产证、购房发票、契税、房产评估报告（附评估公司执照及资质证书）；如
有贷款，提供贷款协议、最新余额证明
4） 股票：股票交易卡、对账单及/或交易记录
5． 作为高级管理者的管理经验
申请人需要对其作为管理者以及该阶段收入成为其重要启动资金的来源（包括投资、购
买房产以及存款）的工作经历提供证明信（推荐信和收入证明合并），必须包含以下内
容：
1） 对于目前存在的公司，必须使用公司抬头纸，包含公司名称、地址、电话、网址（如
有）等信息
2） 公司性质、成立时间、经营范围、员工人数等
3） 申请人的部门、职位、工作职责、工作内容、工作表现、荣誉等
4） 申请人的收入情况（明确到工资、津贴、提成、奖金等）、奖金或提成的计算方法和
依据
作为证明信的支持材料，申请人可以提供劳动合同、任命书、关于奖金或提成的分配依
据文件、收入凭证、企业宣传册等。
关于申请人的收入状况证明，如果申请人目前仍然在职，必须提供公司的审计报告证明
企业的财务状况和经营业绩；如果申请人目前已经离职，可以提供公司的审计报告，也
可以提供业务合同来替代。
6． 作为股东拥有企业的管理经验
1） 营业执照：必须提供目前最新的营业执照副本，以及公司成立时或者申请人成为股
东时的营业执照副本
2） 税务登记证：国税、地税
3） 中外合资/外商独资企业：必须提供商务部的批准证书公证
4） 验资报告：公司成立时或者申请人成为股东时的验资报告，以及增资验资报告
5） 转股或股东变更：工商局盖章的股份转让协议、董事会/股东会决议、章程修正案
6） 对于实物或非货币投资，必须提供相应的资产评估报告
7） 公司的审计报告：最近5 年的审计报告，必须包含以下内容：
a) 每一年的资产负债表、损益表、利润分配表
b) 注册会计师对信息可靠性的意见
c) 会计方法等的附注，包括但不限于主要原则和方法、销售额入账、存货估算、
固定资产及其折旧的确认
d) 其他方面的附注：包括但不限于非主营业务收入、长期投资、无形资产、资本
供给、适用的税收政策等（免税也必须在附注中说明）
e) 会计师事务所的营业执照和详细的联系方式
f) 以上财务报表必须通过提供相关的第三方文件来证实公司最近财务年度的经营
状况，包括最近1 年的税单或者由税务局出具的纳税证明（说明当年的纳税额）；
如果无法提供企业所得税，则必须提供大量的业务销售合同、公司的银行对帐
单
8） 个体户的审计报告：最近5 年的审计报告，必须包含以下内容：
a. 经审计的损益表
b. 中国注册资产评估师出具的个体户资产净值评估
c. 截至面试补料前至少有3 个月公司及申请人银行对账单
7． 承包
1） 承包合同：最初及最终更新的承包合同，以及证明代表发包方签字人权限的相关文
件
2） 发包方的营业执照公证（及承包单位的营业执照公证），以及关于承包的官方文件
3） 对于承包已经终止，必须提供以下文件：
a. 发包方的证明信，说明公司的历史、改制、经营范围等
b. 承包期间承包单位的损益表审计（一般为5 年，如果工程数量不多，而持续时间
较长，建议可以按照项目做损益表审计）
c. 承包期间的相关文件证实承包单位的经营状况
d. 签字人的目前联系方式
e. 对于建筑行业的承包，承包单位的经营状况必须提供每一个项目的完工证明和官
方审核的资产负债表公证
4） 对于承包继续有效：必须提供以下文件：
a. 承包单位独立注册，必须提供近5 年的审计报告，按照上述第7 条第7）款执行
b. 承包单位非独立注册，必须提供近5 年的损益表审计
5） 对于改制企业，必须提供以下文件：
a. 改制申请和官方批准证明公证
b. 改制时的资产评估报告
8． 接受捐赠或遗产的申请人
1） 捐赠：捐赠人身份证明以及进行捐赠的财务能力证明（根据捐赠的金额参照上述第
5-7 项要求提供相应文件）
2） 遗产：死亡证明、遗嘱公证、法院判决等，以及证明资金转予继承人的文件

In Dec. 1998, I invested RMB500,000 Yuan, originated from the accumulations of the previous
jobs, to establish Ameri-Can International Capital Inc., accounting for 10% of the shares. The

In Apr. 2004, I mortgaged the above mentioned property for RMB1,340,000 Yuan, which was
repaid off on Apr. 2008.
